How are Services Provided? All Treatment Provided Via Phone and e-mail after an initial in office session

The first session is a nutritional assessment which is designed to gather information about the specific eating disordered behaviors, current foods eaten, food preferences, goals, and related medical conditions. During this session we will work to design a care plan for you. Subsequent phone appointments will be scheduled based on the treatment goals agreed upon by the patient and the dietitian. Services include Nutritional Assessment, Meal Planning, Weight Management, Menu Planning, Nutrition Education and Dietary Counseling, and Ongoing Support to meet specified goals.

If necessary, you will be asked to sign a consent form allowing the dietitian to contact your physician and/or therapist to send, receive and discuss necessary information regarding your care as needed. All personal and treatment information is confidential. If you don't have a therapist or physician, I will suggest ways to find one. All HIPPA requirements and policies are met.

 

What are the Fees?

For On-going Nutritional Treatment:

The initial consultation is $140.00 and lasts approximately 80 minutes.The fee includes contacting your physician or therapist to discuss your care as needed.

Follow-up sessions are $70.00 and lasts approximately 20 minutes. The fee includes contacting your physician or therapist to discuss your care as needed.

 

For individualized menus, recipes and shopping lists:

Individualizing menus, recipes and shopping lists can be done to fit your Medical Nutrition Therapy Prescription from your physician and also to help you meet your own nutritional goals and your food preferences. Menus can be very detailed, or more specific, depending on the needs of the individual. Contact us for an appointment and we will make this part of our goal of services to you.
